1 serving of chopsuey

1 serving fish fillet (approx. 4 pcs.) with sweet and sour sauce

1 cup of Rice,1 pc. of orange,1 bottled drink (350ml)

PM Snacks

1 serving of eggpie freshly baked

1 bottled iced tea (235 ml)

 

*Inclusive of venue/ accommodation with free use of projector, screen, 2 microphones

*Note: Observing the guidelines on food safety standards, the caterer shall provide the following:

  1. Sanitation Certificate from the City/Municipal Health Office;
  2. Medical Certificate of Catering Service Crew & kitchen staff (servers, food handlers, drivers);
  3. Disclosure on safety and precautionary measures against COVID-19, i.e., wearing of face masks, hairnet, disposable gloves and closed shoes. 4. An additional buffer per meal that is good for 1 pax shall be provided by the supplier for food testing and evaluation purposes.                                                

 

The Schools Division Office of San Carlos City, Pangasinan, hereinafter referred to as the procuring entity, through its Bids and Awards Committee (BAC), now invites qualified suppliers/bidders to quote your lowest price inclusive of VAT on the above-mentioned project, subject to the Terms and Condition stated in the Request for Quotation (RFQ). Duly signed and sealed envelopes compliant with the requirements mentioned in the RFQ may be submitted at the Division Office not later than October 21, 2022, 09:00 A.M.

The BAC shall proceed with the opening of envelopes following the provisions of Small Value Procurement, and upon receipt of at least one (1) quotation on the date of deadline of submission.
